Below you find a copy of a blank "Incident Report" for you to print out and fill in any time a CP Supervisor or Executive violates the NLRA. These "incidents" are called "Unfair Labor Practices" (ULP's) and can have a direct impact on the election results.

Please, do NOT hesitate to print out and fill in then mail the report to the TWU any time you think a violation has occured.
